General Information


Immunizations & Well Visit Schedule:

The following schedule is provided for your convenience and includes recommended well visits and immunizations.

Hospital — Hep B
2 Weeks — Well visit
1 Month — Well visit, check on baby’s growth
2 Months — DTAP, IPV, HIB, Hep B, Prev, and RotaTeq
4 Months — DTAP, IPV, HIB, Prev, and RotaTeq
6 Months — DTAP, IPV, HIB, Prev, and RotaTeq
9 Months — Hep B
12 Months — MMR, Varicella (Chicken Pox), and Hep A
15 Months — HIB and Prev
18 Months — DTAP
24 Months — Hep A
3 Years — Well Visit
4 Years — DTAP, IPV, MMR, and Varicella
11-18 Years — Tdap Booster, Menactra, Trumenba and Gardasil Vaccine

Yearly visits are recommended by the American Academy of Pediatrics. We will inform you of other immunizations as they become required.

There are many new Medicaid plans and insurance companies. Please feel free to call our office before your appointment if you are unsure if our office is in the network.

Prescriptions and Refills:

To request a prescription or refill, please call the office twenty-four [24] hours in advance with the name of the medication and pharmacy number.

Please request medication prescriptions during regular office hours so that your records are available for reference. Records are not accessible after hours or on weekends for the provider on call. We will not call in routine prescription refills after hours or on weekends. Narcotic prescriptions will not be filled by telephone.

Planning ahead will help you receive medication as efficiently as possible. It is easiest to make refill requests during your scheduled visit. If you have not seen your physician in over a year, you will need to make an appointment to receive scheduled medication.
